Patients are typically referred to home health services by a physician following a change in “functional status.” Functional status is measured by one’s ability to perform essential activities of daily living, such as walking or transferring.
Also, patients with new medications, new diagnoses, and those who require health education regarding their health will need home health care. A referral may be made after experiencing a significant medical event or change, including, but are not limited to:
- Neurological incidents
- Wound Care
- Cardiovascular disease
- Orthopedic events
- Surgery
- Generalized weakness
- Diabetes
